NU-SAF-109 — CONFINED SPACE: VAULTS, MANHOLES & HANDHOLES

THE GATE

Nobody puts any part of his body into a vault, manhole, or enterable handhole — not a head to look, not a leg to brace — until the air is tested, the permit is signed, and an attendant is standing topside. Until Nova meets the program requirements in §9, that means: NOBODY ENTERS, PERIOD. Do the work from topside or call the Supervisor.

This is not a big-plant problem that doesn't apply to us. People die in telecom vaults exactly like the ones Nova opens — quiet holes beside quiet roads that looked empty and were full of gas. The air in a hole is the one hazard you cannot see, smell reliably, or tough out.


1. WHAT COUNTS (and what doesn't)

Under 1926.1202, a confined space is a space that meets all three:

  1. Big enough for a man to bodily enter and work in;
  2. Limited or restricted way in and out;
  3. Not designed for people to stay in.

Nova's confined spaces: telecom vaults, manholes, and any handhole big enough for a man to climb or lower himself into. Deep excavations and pits can also qualify — a bore exit pit with a man down in it is Subpart P territory first (NU-SAF-107 when built), but if it's hard to climb out of and could hold gas, treat it under this document too.

What does NOT count: a small handhole you can only reach into. If a man's body cannot fit through the opening, it is not a confined space, and reaching an arm in to grab a pull line is not confined space entry. That is most of Nova's daily handhole work, and this SOP does not slow it down.

But hear the flip side: for any space that IS a confined space, entry happens the moment any part of your body breaks the plane of the opening (1926.1202). Leaning your head into a manhole "just to look" is an entry. That is exactly how the fatalities start — the look, then the slump, then the fall.


2. THE NOVA DEFAULT RULE — DON'T ENTER; ENGINEER THE ENTRY OUT

For a crew our size, the cheapest and safest confined space program is the one where nobody goes in the hole.

Before anyone even asks about a permit, answer this question on the JSA: can the work be done from topside?

If the answer is "the work truly requires a man in the space," the full procedure below applies — and until §9 is closed out (meter, training, retrieval gear, named roles), the answer is Nova does not enter. Call the Supervisor. Rescheduling a task is free. This is Stop Work Authority (NU-SAF-100 §4) and no client schedule overrides it.


3. PERMIT-REQUIRED DETERMINATION (the honest shortcut)

The standard (1926.1203) requires a competent person to evaluate each space and decide whether it is permit-required — meaning it has, or could have, a hazardous atmosphere, an engulfment hazard, or another serious hazard.

Here is Nova's determination, made once, in writing, right here:

Every vault, manhole, and enterable handhole Nova touches is treated as a PERMIT-REQUIRED confined space. No exceptions, no field downgrades.

Why we don't evaluate hole-by-hole:

The standard allows a lighter "alternate procedures" path (1926.1203(e)) when monitoring data proves the only hazard is atmospheric and ventilation alone controls it. Nova may adopt that path later, once we have a meter and months of logged readings behind us. Today it is full permit or no entry.

Working in someone else's space: most vaults Nova opens belong to a utility or a client. The standard (1926.1203(h)) makes information flow both ways — before entry, ask the owner or GC what they know about the space and its hazards, and tell them Nova's entry plan. One question on the phone: "anything we should know about this vault?" Log the answer on the permit.


4. ATMOSPHERIC TESTING — THE 4-GAS METER

Testing is done with a calibrated 4-gas meter reading oxygen (O2), combustible gas (LEL), carbon monoxide (CO), and hydrogen sulfide (H2S). No meter on site = no entry. Not "we'll crack the lid and smell it" — H2S kills your sense of smell before it kills you, and methane has no smell at all.

4.2 How to test (before the lid is even fully off)

  1. Open smart. Use a lid hook, stand upwind, don't put your face over the opening. The first lungful of a bad hole comes out when the lid moves.
  2. Test before entry, top to bottom. Lower the meter (on its pump hose or a cord) and read at the top, middle, and bottom of the space. Gases stratify — methane rises, H2S and propane sink. A clean reading at the collar proves nothing about the floor. Give the meter time to draw and settle at each level.
  3. Test in this order: oxygen first, then combustible (LEL), then toxics (CO, H2S). The meter reads all four at once, but you evaluate in that order — the LEL sensor can't be trusted if oxygen is out of range.
  4. Acceptable air, all four at once:

These are the standard alarm setpoints the meter ships with. Nobody changes a setpoint, ever.

  1. Any fail: nobody enters. Ventilate (§5), retest, and if it won't come clean and stay clean — close the lid, cone it (NU-SAF-105 rules for open structures apply), and call the Supervisor. A hole that keeps going bad has a supply feeding it, and that is a utility-owner and possibly a 911 problem, not a crew problem.
  2. Continuous monitoring during entry. The meter goes in the hole with the entrant, or hangs in the space at breathing height, running, the entire entry. Not a spot check every hour — continuously. Air that tested clean at 9:00 can be lethal at 9:20. Meter alarms = everybody out, immediately, no discussion, and you don't go back in until it's retested clean and the reason for the alarm is known. Any alarm or evacuation also opens an incident record per NU-SAF-121 — same day.
  3. Log every reading on the permit — pre-entry readings with times, and periodic readings during the entry.

Every entry has a dedicated attendant (1926.1209) at the opening, full time:


7. RESCUE = NON-ENTRY RETRIEVAL (the section that keeps two families whole)

Here is the statistic this whole section exists for: roughly 60% of confined space deaths are would-be rescuers (NIOSH). The pattern never changes — a man goes down, his partner sees him drop, climbs in after him, and breathes the same air that dropped the first man. Now it's a double fatality, and the second one was 100% preventable. The gas that took down a healthy man in seconds will take you down in seconds. Going in after him is not brave; it's how one funeral becomes two.

So Nova's rescue plan is built so that rescue never requires a body in the hole:

  1. Every entrant wears a full-body harness with a dorsal (upper-back) D-ring, attached to a retrieval line, before entry (1926.1211). Not stored on the truck — worn, clipped, before his boots leave the surface.
  2. For any vertical entry deeper than 5 feet, the retrieval line runs to a mechanical retrieval device — tripod and winch, or davit — set over the opening. A man cannot dead-lift an unconscious man out of a hole by rope; the winch can.
  3. Rescue sequence: attendant orders out / sees the entrant down → calls 911, says "confined space emergency" → cranks the entrant out on the retrieval system → starts first aid topside. At no point does anyone go in.
  4. The only exemption to rigging retrieval is when the equipment itself would increase risk or couldn't work (a line that would snag in an obstructed space). That is a Supervisor call made before entry and written on the permit — never a field improvisation after.
  5. 911 response is part of the plan, not all of it — by the time a fire crew arrives, retrieval must already be done. The winch is the rescue. 911 is the ambulance.

8. GAS STRIKE NEARBY = EVERY HOLE IS A SUSPECT HOLE

A gas strike changes the ground itself. Escaping gas doesn't just vent at the strike — it travels through soil, follows conduit paths and utility trenches, and collects in every underground void in the area. After any gas hit, or anytime you smell gas or the locator work suggests a leak:

The same logic applies in reverse: if a vault tests high on LEL with no strike on your job, something is leaking near you. Report it to the utility — you may be the first one to know.


FIELD CARD — CONFINED SPACE (Tier 3 · print this section)

FIRST QUESTION: Can you do it from topside — reach tool, camera, rig the pull to surface? Then do that and skip this card.

Body doesn't fit through the opening? Not a confined space. Reach in and work. (Head or limbs into a space a body CAN fit through = that's an entry. Don't.)

A man must go in:

  1. STOP — is the program live? No meter, no harness/winch, no trained attendant = NO ENTRY. Call Supervisor.
  2. Permit form NU-SAF-109-F started. Ask the space owner what they know about the hole.
  3. Lid off smart — hook, upwind, face out of the opening.
  4. Meter (bump-tested today): test top / middle / bottom — BEFORE the blower starts. O2 19.5–23.5 · LEL <10% · CO <35 ppm · H2S <10 ppm.
  5. Blower on, fresh-air intake upwind of any exhaust — then RETEST all three levels. Any fail = no entry; won't clean up = lid on, cone it, call Supervisor.
  6. Entrant in harness, retrieval line clipped to dorsal D-ring. Deeper than 5 ft = tripod/winch rigged over the hole.
  7. Attendant posted. Does nothing else. Never enters — not even for rescue.
  8. Meter stays in the hole, running, whole entry. Alarm = everybody out.
  9. Man down: 911 ("confined space emergency") → winch him out → first aid topside. Nobody goes in after him.
  10. Done: entrant out, count confirmed, permit closed and signed, readings logged, lid back on.

GAS HIT OR GAS SMELL ANYWHERE NEARBY: every hole is a gas hole. Everybody out of everything. NU-SAF-110 / HDD Cards 2 & 16.

9. WHAT THIS DOCUMENT NEEDS FROM ADAM

  1. The 4-gas meter question (§4): does Nova own one? If not, buy one (~$400–800) plus test gas — this is the purchase the entire SOP hangs on. Then name who owns bump/calibration logging.
  2. The retrieval gear question (§7): tripod/winch + full-body harnesses with dorsal D-rings — own, buy (~$1,200–2,500), or keep vertical entry prohibited and written that way.
  3. Name the roles: who is the competent person for space evaluation, and which crew members get trained as entrant / attendant / entry supervisor (1926.1207 requires training before assignment). Until named and trained, the §2 no-entry default stands.
  4. Add the points classes to NU-PPL-403 (proposed): entering without permit/testing = Class C (6 pts), repeat = Final; attendant leaving post or entering the space = Final; meter not bump-tested = Class A (1 pt). Not enforceable until they're in 403.
  5. Decide whether Nova ever needs entry at all. If the honest answer across current contracts is "topside covers 100% of our vault work," say so — then this SOP's job is to keep it that way, and gear purchases can wait for the contract that changes the answer.
  6. On approval, update NU-GOV-000: move NU-SAF-109 from the §4.1 gap list to the issued table.
